Children's Medical Services
The Children's Medical Services (CMS) program provides children with special
health care needs with a family centered, managed system of care. Children
with special health care needs are those children under age 21 whose serious
or chronic physical, developmental, behavioral or emotional conditions require
extensive preventive and maintenance care beyond that required by typically
healthy children.

to fulfill an unmet need in Florida: the need for information. The Family
Cafe is proud to offer educational opportunities and information on meaningful
topics for individuals with disabilities and their families in a family-centered
environment. We hope this information will help individuals with disabilities
make sensible and educated decisions regarding the services they receive.

This site includes a Health Services Directory which young adults can use to find health care providers in their area and a health care transition Tool Box that is filled with helpful documents and links to local, state and national resources.

Family Involvement
FIFI is a non-profit organization with a 501C3 status for the purpose of enhancing,
facilitating, and supporting family and consumer involvement in the development
of responsive, family centered, and community based systems of care.
- Family Voices Florida
Family Voices is a national, grassroots clearinghouse for photo of mother
and daughterinformation and education concerning the health care of children
with special health needs.
- Family Network
on Disabilities of Florida, Inc.
FNDF is a statewide network of families and individuals who may be at-risk,
have disabilities, or have special needs and their families, professionals,
and concerned citizens. Their mission is to ensure that Floridians have full
access to family-driven support, education, information, resources, and advocacy.

Council
We encourage and advocate opportunities for persons with developmental disabilities
and their families, to enhance their quality of life within their communities.
We support innovative initiatives that demonstrate new approaches to enhance
the independence, productivity, inclusion and self-determination of individuals

by the Florida Developmental Disabilities Coucil and produced by the Transition

at the Institute for Child Health Policy, University of Florida
The mission of the Health Care Transition Initiative at the University of
Florida is to increase awareness of, gain knowledge about, and promote cooperative
efforts to improve the process transitioning from child-centered (pediatric)
to adult oriented health care. Our vision is to improve the transition process
for all adolescents and young adults, although our current efforts focus on
those with disabilities and special health care needs.

The Lance Armstrong Foundation (LAF) believes that in the battle with cancer,

to live strong. Our mission is to inspire and empower people with cancer to
live strong. We serve our mission through education, advocacy, public health
