    The Children's Medical Services (CMS) program provides children with special health care needs with a family centered, managed system of care. Children with special health care needs are those children under age 21 whose serious or chronic physical, developmental, behavioral or emotional conditions require extensive preventive and maintenance care beyond that required by typically healthy children.

  • Catholic Charities Bureau, Inc. Jacksonville Florida
    The mission of Catholic Charities is to provide services to anyone in need, regardless of race or religion; to advocate justice, human dignity and quality of life; and to call all people to join in these efforts; thereby reflecting the compassion of God in Christ.

  • Counseling Support Jewish Family and Community Services

    The Family Café, Inc., a non-profit statewide organization, was created to fulfill an unmet need in Florida: the need for information. The Family Cafe is proud to offer educational opportunities and information on meaningful topics for individuals with disabilities and their families in a family-centered environment. We hope this information will help individuals with disabilities make sensible and educated decisions regarding the services they receive.

  • Florida Health and Transition Services (FloridaHATS)
    This site includes a Health Services Directory which young adults can use to find health care providers in their area and a health care transition Tool Box that is filled with helpful documents and links to local, state and national resources.

  • Florida Institute for Family Involvement
    FIFI is a non-profit organization with a 501C3 status for the purpose of enhancing, facilitating, and supporting family and consumer involvement in the development of responsive, family centered, and community based systems of care.

  • Family Voices Florida
    Family Voices is a national, grassroots clearinghouse for photo of mother and daughterinformation and education concerning the health care of children with special health needs.

  • Family Network on Disabilities of Florida, Inc.
    FNDF is a statewide network of families and individuals who may be at-risk, have disabilities, or have special needs and their families, professionals, and concerned citizens. Their mission is to ensure that Floridians have full access to family-driven support, education, information, resources, and advocacy.

  • Florida Developmental Disabilities Council
    We encourage and advocate opportunities for persons with developmental disabilities and their families, to enhance their quality of life within their communities. We support innovative initiatives that demonstrate new approaches to enhance the independence, productivity, inclusion and self-determination of individuals with developmental disabilities in all facets of community life.

  • Florida Developmental Disabilities Council - Transition Training
    Transition training designed for youth and families with special needs, commissioned by the Florida Developmental Disabilities Coucil and produced by the Transition Initiative at the University of Florida's Institute for Child Health Policy.

  • Florida Division of Vocational Rehabilitation
    An employment resource for businesses and people with disabilities. Our mission is to enable individuals with disabilities to obtain and keep employment.

  • Health Care Transitions at the Institute for Child Health Policy, University of Florida
    The mission of the Health Care Transition Initiative at the University of Florida is to increase awareness of, gain knowledge about, and promote cooperative efforts to improve the process transitioning from child-centered (pediatric) to adult oriented health care. Our vision is to improve the transition process for all adolescents and young adults, although our current efforts focus on those with disabilities and special health care needs.

  • Institute for Community Inclusion
    The mission of the Institute for Community Inclusion is to support the rights of children and adults with disabilities to participate in all aspects of the community.

  • Paul V. Sherlock Center on Disabilities
    The mission of the Paul V. Sherlock Center on Disabilities is to promote the community membership of individuals with disabilities in school, work and society.

  • Royal Children's Hospital Transition Project
    Transition of patients to adult health care, information for patients, parents, and health care profesionals.

  • Livestrong
    The Lance Armstrong Foundation (LAF) believes that in the battle with cancer, unity is strength, knowledge is power and attitude is everything. Founded in 1997 by cancer survivor and champion cyclist, Lance Armstrong, the LAF provides the practical information and tools people living with cancer need to live strong. Our mission is to inspire and empower people with cancer to live strong. We serve our mission through education, advocacy, public health and research programs.

    We provide information and connections to health and transition expertise nationwide – from those in the know, doing the work and living it!

  • DHMC STAR Program
    Steps Toward Adult Responsibility - The STAR Program promotes healthy psychological and social development for adolescents with a chronic physical health condition.

  • Dream Team
    The DREAM Team web site provides resources for young people with conditions like arthritis, their families and the health professionals who look after them.
